What is the MVM Convenient Care Program?

We are a primary care practice dedicated to providing high-quality healthcare services to our Island community. Because there are no urgent care clinics on Martha’s Vineyard, we created the Convenient Care Program, as an addition to our primary care practice, so that you can get the help you need when you need it.

Convenient Care Fees apply only to our non-primary care patients. These fees support:

  • Quality of Care: The revenue generated from these fees allows us to maintain high standards of care, invest in modern medical equipment, and attract skilled healthcare professionals.

  • Operational Sustainability: These fees help cover operational costs, ensuring that our facility remains open and functional.

How much are your Convenient Care Fees?

  • Weekly: $150

What is Continuous Care Management (CCM) and how does it work?

CCM service is designed for patients with two or more chronic conditions (such as diabetes, hypertension, or heart disease) that are expected to last at least 12 months. Your personalized continuous care plan will be created just for you! If needed, we will provide you with a monitoring device, deliver it to your home, and instruct you how to use it. You can use this device to measure your vitals and share them with your care team. A member of your care team will reach out to you for a meeting to discuss these results each month.

Who is on my CCM team?

Your care team will consist of a pharmacist, nurse, and nutritionist who will work in conjunction with your primary care provider to provide medications, track and monitor your at home medical devices, and coordinate any adjustments that need to be made to optimize reaching your health goals.

How do I sign up for CCM?

To sign up for CCM, call our pharmacist, Amy, at 478-227-0684.
