What is the MVM Convenient Care Program?
We are a primary care practice dedicated to providing high-quality healthcare services to our Island community. We understand that you may have a primary care provider elsewhere but need medical attention while you are on Island. Because there are no urgent care clinics on Martha’s Vineyard, we created the Convenient Care Program - as an addition to our primary care practice - so that you can get the help you need when you need it.
We have implemented Convenient Care Fees for our non-primary care patients only. These fees support:
Quality of Care: The revenue generated from these fees allows us to maintain high standards of care, invest in modern medical equipment, and attract skilled healthcare professionals.
Operational Sustainability: These fees help cover operational costs, ensuring that our facility remains open and functional.
In order to make a Convenient Care appointment you will be charged a Convenient Care Fee.

What is Continuous Care Management (CCM)?
Under continuous care, you'll be assigned a care team that you can contact 24 hours a day, 7 days a week, via email or phone. You'll also be able to make routine appointments with your primary care provider.
How does CCM work?
Your continuous care plan will be created according to your needs! If it’s needed, we will provide you with a monitoring device, deliver it to your home, and instruct you how to use it. You can use this device to measure your vitals and share them with your care team. A member of your care team will reach out to you for a meeting to discuss these results each month.
Who is on my CCM team?
Your care team will consist of a pharmacist, nurse, and nutritionist who will work in conjunction with your primary care provider to provide medications, track and monitor your at home medical devices, and coordinate any adjustments that need to be made to optimize reaching your health goals.
How do I sign up for CCM?
Call our pharmacist, Amy, at 478-227-0684.
